If you would like to sign up for service, or if you were reffered to this page via phone by one of our representatives, you must fill out the appropriate service contracts. Just print them out, read them carefully, fill them out to the best of your knowledge, and bring them in or mail them to us at P.O. Box 753, Hereford, TX, 79045.
Master Electric Service Agreement (PDF) -The MEMBER hereby makes application and
agrees to purchase electric service from DEAF SMITH ELECTRIC COOPERATIVE, INC. upon the following terms and conditions.
Meter Agreement (PDF) -The purpose of this agreement is to serve as an addendum to the Master Electric Service Agreement currently in force between the DEAF SMITH ELECTRIC COOPERATIVE, INC. and the MEMBER.
Irrigation Meter Agreement (PDF) -The purpose of this agreement is to serve as an addendum to the Master Electric Service Agreement currently in force between the DEAF SMITH ELECTRIC COOPERATIVE, INC. and the MEMBER.
Hold Harmless Agreement (PDF) -For and in consideration of the installation of overhead and underground electrical line, the Consumer agrees to be responsible for and pay all damages incurred to underground water, gas, and communication lines upon premises and to hold harmless the Cooperative from the payment and repair thereof. Consumer agrees to designate the location of overhead and underground electrical line in an effort to minimize such damages. The Cooperative agrees to use due diligence in the installation of electrical lines
Tax Exemption Certificate (PDF) -I understand that I will be liable for payment of Sales Tax which may become due for failure to comply with the provisions of the State, City and/or Metropolitan Transit Authority Sales and Use Tax Laws and Comptroller rules regarding exempt purchases. Liability for the tax will be determined by the price paid for the taxable items purchased.
